What do you want to keep doing in your next 5–10 years?

The trip you keep talking about. Time with family. Everyday moments you want to keep saying yes to.

At DHC Precision Outcomes Medicine, we begin by listening to what matters to you. Our standalone four-week Precision Health Assessment brings physician guidance to six health categories and three practical priorities. After your results review, Precision Living is an optional, separate choice.

Your next chapter deserves a clearer starting point.

You may know what you want from the years ahead without knowing where to begin with your health. We begin with your questions, your priorities, and the life you want to lead.

Your four-week Precision Health Assessment comes first. It is a complete, independent service: time to understand your health, review your findings with your physician, and identify three practical priorities based on your goals.

After reviewing your assessment results, you may separately choose Precision Living for an additional eleven months of physician-guided care, including home visits. Continuing is optional.

A short film

Your Next 5 to 10 Years

Six perspectives. One person.

Your goals, daily routines, and clinical findings belong in the same conversation. We consider six connected areas of health.

NutritionYour food, hydration, and everyday eating routines.
MetabolicBody composition, energy use, and your response to a meal.
FitnessStrength, balance, and physical capacity, assessed as appropriate.
Sleep & restorationSleep patterns, recovery, and daily rhythms.
CognitiveAttention, memory, and concerns you want to discuss.
ClinicalYour medical history, examination, and physician interpretation.

We review the findings together. An assessment score is not a diagnosis.

Concierge Programs: begin with your assessment

Your four-week Precision Health Assessment evaluates your health across six categories, providing a comprehensive starting point. Together, we review your findings and identify three practical priorities based on your goals.

The assessment is a complete, independent service.

01

Step 1

Begin with your goal

Tell us what matters to you and what you want to keep doing. Your history and physician’s judgment help guide the assessment.

02

Step 2

Measure what is relevant

Selected assessments bring together information about your health. A physician home visit and Pantry Walk connect that information to your actual foods and routines.

03

Step 3

Review your starting point

Meet with your physician to discuss your findings, understand your Health Score, and identify three initial priorities.

04

Step 4

Choose your next step

The assessment is a complete, independent service. After reviewing your results, you may separately choose Precision Living: an additional eleven months of physician-guided care, including home visits.

DHC Doctors House Call™

Health changes have to work where you live.

What is in your pantry? How does a typical day unfold? Which routines support your goals, and which are difficult to change?

A physician home visit and Pantry Walk bring those everyday details into the assessment. Together, we look at practical priorities in the context of your home and your life.

This approach is inspired by traditional house calls, as Dr. Kelly grew up in the home of a family practice physician who was deeply loved by his patients and known for his dedication to caring for entire families. As a child, he witnessed firsthand how medicine changed when it was delivered in the home, where trust was deeper, relationships were stronger, and care was more personal and complete.

Understand your results. Know where to begin.

Measurements become more useful when you understand what they mean for you.

Your physician reviews your findings alongside your history and goals, explains your starting point, and helps identify three priorities. If you continue with care, repeat measurement provides opportunities to review changes and adjust the plan.

DHC Coordinates the Right Next Step.

DHC does not claim to do everything. When the PHA identifies a medical or structural problem that requires specialty evaluation, DHC helps coordinate the right next step. This is central to Precision Outcomes Medicine: identify the barrier, correct what can be corrected, then measure recovery and improvement.

In Loving Memory

A Dedication

DHC Doctors House Call™, Precision Outcomes Medicine is dedicated to the memory and legacy of Nathan L. Poff, M.D., Dr. Kenneth F. Kelly’s stepfather.

Dr. Poff was a family practice physician who believed deeply in the responsibility of caring for patients in their own homes. Through his house calls, he brought medicine beyond the walls of the clinic and into the place where life actually happens. Here, trust is built, families are known, and healing becomes personal.

As a young boy, Dr. Kelly witnessed these house calls firsthand while growing up with Dr. Poff. He saw the relationships Dr. Poff built with patients, the respect he earned through presence and consistency, and the quiet impact of medicine practiced with dedication and humanity. That experience shaped a lifelong understanding that true healthcare is not transactional; it is relational, continuous, and grounded in trust.

This model of care, now carried forward as DHC Doctors House Call™ within Precision Outcomes Medicine, is a continuation of that philosophy in a modern form, combining home-based evaluation, structured physical examination, and data-driven outcomes tracking.

A Legacy of House Calls

“A place of honor is held within the practice for Dr. Poff’s original leather medical bag and historical photographs of his house calls, preserving the legacy that inspired this work.”

This dedication stands as a recognition that the future of medicine is built not only on innovation, but also on remembering what made care meaningful in the first place.
